Don't let the cost of prescriptions or medical devices keep you from seeking services to get well. We are committed to providing all patients with excellent care. Having access to medications and medical devices is essential to quality care.

Qualifying patients can receive free or low-cost medicine and medical devices. Uninsured or underinsured patients may still quality for current treatment plans.

To see if you qualify for the program, you need to fill out an authorization form during a doctor's office visit. You can get the patient assistance program authorization form from:

• patient financial application representative
• social workerfinancial counselor

SSM Health will call you to verify your enrollment in the program. Be prepared to provide your most recent tax return, W2, and any health insurance. If you qualify, your free or discounted medicine or medical device will be reflected on your bill.
